Survey System Quotations
 
Survey Plus quotations are FREE to both members and non-members. Non-members must first provide the system with basic customer information. Existing customers can add new quotations directly.

  • Existing Members
    An existing customer login is provided for ease of use. Existing customers can quickly and easily login and proceed to the quotation data entry page.

  • Non-Members
    Non-members or those who wish to become customers are provided with an area on this page to enter basic pertinent customer information and then proceed to the quotation data entry page.
     
Below are examples of the login and "Company Profile" data entry pages.
 

 
 
Login Page

Both existing and new customers who have submitted quotations will use this login screen to view the Survey System "Home Page". Existing customers can quickly and easily login and proceed to the quotation data entry page.

The Survey System Quotation pages appear as follows:
 

  • User ID
    Here you enter the User ID you select during your first quotation data entry or the one selected or changed using the "User Profile" page.

  • Password
    Here you enter the Password you select during your first quotation data entry or the one selected or changed using the "User Profile" page. After entering both your User ID and Password you click the "Login" button to proceed to the "Home Page".
     

 
 
Company Profile Page

The first time you request a FREE quotation you will need to enter your basic company/user information into the system using this "Company Profile Page". This page displays if you are a non-member and select a quotation link.

  • Required Fields
    Those data entry fields on this page that are followed by the designation "Required" in red necesitate a proper entry.

  • State Selection
    Select your state from the dropdown list. If your state is not in the list, select "Other".

  • The "Next" Button
    After entering your information in all the applicable fields, click the "Next" button to proceed to the "Quotation Form".
     

 
 
The Quotation Form
 
The following two pages show the forms that are used to enter the quotation information required to submit a quotation request:
 

 
  • Company Name, Contact/User Name, Email
    These fields are automatically populated by the system with data that should have already been entered.

  • User ID, User Password
    These two fields only appear for those quotation requests to be submitted by non-members. These are required in order to establish a User ID and Password for later use in the login procedure. This will allow you to obtain the status of the quotation request and other information when re-entering the survey system. It is recommended that you use your email address as a User ID. This will insure that your ID will be unique and also make it easy to remember.

  • Title
    This will be the Title for your survey and survey quotation. This will serve as a primary identifier for use in future referencing of the survey information.

  • Start Date, End Date
    This will be the starting and ending date that will establish the period in which your survey will be available to the potential respondents. Please allow enough lead-time from the current date to the start date to receive your quotation and submit your approval.

  • Number Of Emails To Send
    This is the approximate number of potential respondents you wish your survey to reach. This is usually the number of emails you will be sending out to advertise your survey. Keep in mind that the difference between the actual number of respondents and the potential number of respondents notified may vary considerably.

  • Provide Survey Target Market Emails
    If you want Future Access to provide you with an email list of potential respondents for your survey, you indicate this by clicking "Yes" to this selection. If you select "Yes" it will be necessary for you to provide us with an email "Target Market" answer to the question below.

  • Email Mailing Services
    If you want Future Access to provide you with the means to email your list of potential respondents, you indicate this by clicking "Yes" to this selection. This list can be one that you provide us or one, which we provide for you. If you will be providing your own list, you will need to indicate the type of list and the file format of the list in the "Survey Content" space provided below.

  • Email Verification Of Quotation
    If you wish your quotation to be sent to you via email, you indicate this by clicking "Yes" to this selection. You will also be able to access the status and results of your quotation request on this web site after it has been prepared regardless of how you respond to this selection.
     

 
  • Survey Password
    You need to have a password login for your survey (that will be sent with the respondent email). The password will also help distinguish between multiple surveys that may be active simultaneously.

  • Target Market(s)
    Here you enter the "Target Market(s)" you want to reach as respondents to your survey. If you want Future Access to provide you with an email list of potential respondents, you will need to specify the type of list to use by this "Target Market".

  • Survey Budget Amount
    If you have a budgetary limitation for your survey, enter that amount and/or any other budgetary stipulations you might have.

  • Survey Content
    Here is where you enter in the specifics of your survey/poll. You will need to be as complete as possible to help insure that we provide you with all that are needed to make your survey a complete success. You can cut and paste a sample of how you want the survey to look or simply enter the sample here. Be sure to include all the questions, in detail, you want to ask. You will also need to show or explain the each question type (i.e. text field, dropdown, radio buttons, checkboxes, etc.). You may also want to include the introduction text, any instruction text, the response text that displays after the survey is taken, and any other pertinent information. You can also email a sample of the survey to us if it makes it easier. Also, if you are going to provide an email list for us to email for you, you can email the list or describe the type of list and file format here.
